Types of artificial insemination

Homologous IUI (partner sperm): requirements and when it is suitable

Homologous IUI uses your partner’s sperm, which we optimise in our andrology laboratory. It is suitable when the semen analysis shows slight abnormalities but a sufficient motile fraction can be obtained after sperm preparation (sperm capacitation). During the consultation we explain the total motile sperm count (TMSC) clearly and, if it is repeatedly low, we do not waste your time: we discuss other options with you.

Donor IUI: single women, two mums, or heterosexual couples who need it

Donor IUI is ideal for single women, same-sex female couples, or when the partner’s sperm cannot be used (e.g. azoospermia or genetic conditions that cannot be ruled out). We take great care with donor selection, applying strict medical and analytical criteria, and when we support two mums or a woman coming on her own, we pay special attention to communication and timing.

Mild ovarian stimulation vs. natural cycle

IUI can be performed in a natural cycle (no medication) or with mild ovarian stimulation (~10 days). We carry out serial ultrasound scans and, once the follicle reaches the right size, prescribe hCG to trigger ovulation. We tend to favour straightforward, well-explained protocols, with smooth communication and fixed appointment times so you can plan with peace of mind.

hCG and scheduling (38-40 hours)

We coordinate the laboratory and consultation so that the insemination takes place 38–40 hours after the hCG injection — or according to your LH surge if you are on a natural cycle. We give you simple instructions (hydration, medication if applicable) and answer any questions on the spot.

Intrauterine insemination: duration, pain and aftercare

We introduce a thin catheter and deposit the prepared sample inside the uterus. It takes just a few minutes, is painless in most cases (similar to a smear test) and requires no anaesthesia. We recommend going about your normal life; for comfort, you may prefer to avoid intense physical effort on the same day.

Key tests

At your free first consultation with our specialists, we review your medical history and request:

• Ultrasound scan (gynaecological).
• Hysterosalpingography (HSG) or HyFoSy to assess tubal patency (whether the fallopian tubes are open).
• Up-to-date serology and cervical smear.
• Semen analysis (if you have a male partner).
• Other tests at the doctor’s discretion (e.g. ovarian reserve).

How we decide together with you

We translate complexity into clear decisions: if something does not fit with IUI (age, tubes, TMSC, medical history), we discuss it before starting. Our principle is simple: treatments that make sense, and on time.

Sperm preparation in the andrology laboratory

Sperm capacitation and selection

The laboratory prepares the sample to concentrate the sperm with the best motility and morphology. This step is crucial in homologous IUI and is also carried out in donor IUI to ensure the highest quality.

Logistics: partner vs. donor

• Partner: the sample is provided 1–2 hours beforehand, with clear instructions on abstinence and transport.
• Donor: we work with authorised sperm banks; the sample arrives fully validated and we also prepare it in our lab.

Success rates and number of cycles: realistic and by your side

Indicative success rates (homologous / donor IUI)

As a general reference, homologous IUI success rates are around 15–25 % per cycle, and donor IUI around 20–30 % per cycle; age and diagnosis make a significant difference. Some clinics publish cumulative rates over 3 cycles of approximately 46–54 % depending on the sperm source. We share your personalised success rates and review each cycle to decide whether to repeat, adjust medication, or change strategy.

When to move on to IVF?

Although every case is unique, we generally do not chain multiple cycles if the chances are low (advanced maternal age, limited TMSC, questionable tubal patency…). We prefer to look after you, avoid the toll of repeated negative results, and propose IVF (In Vitro Fertilisation) or other alternatives in good time.

Risks and how we minimise them

Multiple pregnancy, ectopic pregnancy and ovarian hyperstimulation syndrome (OHSS)

IUI is a safe procedure. Even so, we monitor the risk of multiple pregnancy (by adjusting medication and cancelling the cycle if too many follicles develop), ectopic pregnancy (with clear warning signs) and ovarian hyperstimulation syndrome (OHSS), which is uncommon with mild stimulation. We explain every scenario so you feel calm and well informed.

Legal framework and safety: what you should know

In Spain, assisted reproduction techniques must be performed at authorised centres using samples from authorised banks. Self-insemination at home is not a safe practice and falls outside the regulated clinical framework. At Love Fertility we provide full traceability and quality controls at every step, so your journey is safe and peaceful.

Two-week wait (TWW), pregnancy test and first scans

Emotional support and post-IUI guidelines

The two-week wait (TWW) lasts approximately 14 days. These can be emotionally sensitive days, which is why you have access to psychological support if you need it. Your doctor will call you with the result and, if it is positive, we schedule a monitoring ultrasound scan 2–3 weeks later to confirm the gestational sac and, subsequently, the heartbeat. If it is negative, we do not leave you on your own: we review the cycle and agree on the plan together.

IUI for all families

We work with single women, same-sex female couples and heterosexual couples. We adapt information, timing and logistics to your family plan. When we support two mums or a woman on her own, we take special care with language and the experience in the consultation room.
